Public healthcare expenditure (ISTAT - SHA) amounts to €130,291 million (2023 data), to which must be added out-of-pocket expenses (expenses incurred directly by citizens), which amount to €40,641 million. The latter mainly concern treatment and rehabilitation, long-term care, ancillary services, the purchase of pharmaceutical products and therapeutic devices, and preventive services. Their impact on overall expenditure is constantly growing: in the last decade, direct expenditure has increased significantly. If we add to this figure the expenditure intermediated by health funds, private expenditure reaches almost 30% of the total. Changes in citizens’ spending habits and the structural difficulties of the national health service – including defunding, inefficiencies and waiting times – are increasingly favouring the use of private healthcare and insurance supplements. This is evidenced by the continuous growth of supplementary funds, mutual aid societies, outpatient networks and diagnostic centres, all of which have made access to services easier and faster. AFFRONTARE il tema delicato degli employee benefits, ossia i benefici che il datore di lavoro mette a disposizione dei propri dipendenti, richiede prima di tutto una visione chiara del contesto sanitario italiano. La spesa sanitaria pubblica (ISTAT - SHA) ammonta a 130.291 milioni di euro (dati 2023), a cui vanno a sommarsi le spese out of pocket (spese sostenute direttamente dai cittadini) che ammontano a 40.641 milioni di euro. Queste ultime riguardano principalmente cure e riabilitazione, assistenza a lungo termine, servizi ausiliari, acquisto di prodotti farmaceutici e apparecchi terapeutici, e servizi di prevenzione. La loro incidenza sulla spesa complessiva è in costante crescita: nell’ultimo decennio l’esborso diretto è aumentato in modo significativo. Se a tale dato si aggiungono anche le spese intermediate dai fondi sanitari, la spesa privata arriva a sfiorare il 30% del totale. Il cambiamento delle abitudini di spesa dei cittadini e le difficoltà strutturali del SSN – tra definanziamento, inefficienze e tempi di attesa – stanno favorendo sempre di più il ricorso alla sanità privata e all’integrazione assicurativa.
